Effective January 1, 2009

Eighth graders can join BBYO!

$75 for Annual Membership Dues

$149 for those joining (or re-joining) for more than two years

Required forms to be submitted to Regional office:

• Annual Participation Authorization (PRO-3)
• Participant Medical Information and Consent to Treat Form

New Members: 2008-2009 Programming Year. Grades 8-12 can join for $75 and gives you annual membership through June, 2009. If you are joining for more than 2 years you can take advantage of the $149. Membership Form, Annual Participation Authorization (PRO-3) and Participant Medical Information and Consent to Treat Form required are to be mailed to Regional Office along with check payment or call the office 908.925.0310 to make a credit card payment or go to www.b-linked.org to join; If necessary, payment by check will be accepted. Above forms still needed to be submitted.

Re-Registration dues for 2009-2010 are $75 Annual Participation Authorization (PRO-3) and Participant Medical Information and Consent to Treat Form to be mailed to the Regional Office. Re-registration can be done on www.b-linked.org with a credit card or by calling the Regional Office 908.925.0310. If necessary, payment by check will still be accepted.
